This Service Request Form is for any customer that currently owns a DCM machine:

  • For those customers whose machine is within warranty, this form is the initial step to gather information in order to quickly diagnose and work with you to get your machine operating at peak capacity.
  • For those customers who either (1) purchased a machine from DCM but the machine is outside of our warranty or (2) purchased a DCM machine from a third party, we are happy to provide an initial complimentary diagnostic phone call after you submit a Service Request Form. If we are unable to diagnose your service needs within that initial call, we charge $60 per hour to work on assessing, diagnosing, and servicing your machine via phone or email. This may be paid via purchase order or by providing DCM a credit card to charge for services rendered.
